Technical assistance is available for parish nurses, health ministries and clergy who have signed faith
partner agreements to have a designated person collaborate with Inova CHP regarding community
linkages to meet their health ministry needs.

- Congregational Needs Assessments Surveys are available for partner congregations to
explore self-reported health concerns for their congregation. This will help shape education and
services provided through their health ministry.
- Community Linkages - CHP staff will share community linkages and resource information
using various approaches they have found available to congregations.
- Individual Technical Assistance is available as requested.
